University Collections Forms

If your Cornell University unit already an established client of University Collections, you may place new accounts by completing the following form:

Note: Please be sure to complete all of the requested information including SS#, address, amount due, and date of service.

If the account placed is for an employee, we will encourage the individual to use payroll deduction to assist them in repayment.
